Referrals

You do not require a referral from your dentist to be seen at our office. We always welcome referrals from friends and family and consider it the greatest compliment when our services are recommended. If you were referred to another periodontist but selected our office instead because we are in-network with your insurance, simply advise your dentist to forward your records and referral to us.

Dr. Indech recognizes that patients with special needs may have a more difficult time getting the dental care they need. These patients are often suffering from undiagnosed periodontal disease and tooth decay as they have difficulty caring for their teeth, see the dentist infrequently and are often not provided the day-to-day care they require by caregivers. Patients with reduced mobility, mental disability and the elderly may require modifications to our normal care. We realize that each patient is a unique individual and deserves evaluation and treatment with a thoughtful and compassionate touch.

What to Bring on Your First Visit

Please assist us at the time of your initial visit to the office by providing the following information:

  • Your referral slip and x-rays from your referring dentist.
  • A list of medications you are currently taking.
    (please note that you can fill out your patient registration and medical history form below).
  • If you have dental insurance, please bring any forms or insurance cards with you to the appointment.
Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.

Please notify the office if you have a medical condition or concern prior to your treatment (e.g. artificial heart valves or joints, heart murmurs requiring premedication, severe diabetes, blood thinners or poorly controlled high blood pressure).

If your referring dentist has taken x-rays, you may request that they be forwarded to us.

On your first visit to our office, we will review your medical and dental history, the reason you have been referred and your expectations for treatment. A thorough examination will be performed followed by a review of our findings and presentation of our treatment recommendations.

Throughout the treatment process, we will communicate directly with your general dentist to advise of our findings and recommendations, update your progress and coordinate your continuing care. As a patient in our practice, we encourage you to call our patient care coordinators with any questions you may have regarding your consultation, treatment plan or treatment.
